Online Pre-Participation Survey & Consent To Treat
One of the valuable services provided by Orthopedic ONE to Classics Eagles, is a pre-participation survey with consent to treat. All families are asked to complete this survey, which will assist Orthopedic ONE with screening athletes for injury risk and also to provide the best possible treatment in the event of injury. Orthopedic ONE is the exclusive sports medicine provider to Classics Eagles and Barça Academy Columbus. The partnership provides services and benefits to enhance the experience for all members of the CE and BA families, including players, families, coaches and directors. The practice has certified athletic trainers on site for set hours on a weekly basis during the season and a 24 hour hotline to respond to any acute injury that may take place, and to provide general first aid and sports medicine assistance. Certified athletic trainers can prevent, assess and treat athletes, as well as direct access to a musculoskeletal physician, when needed.
Under the partnership agreement, services provided by Orthopedic ONE will include but are not limited to the following:

Previous athletic training affiliations include: Charlotte Rugby Club, Hendrick Motorsports Pit Crew, Stewart Haas Racing Pit Crew, various high schools in OH, NC, and IL, Ohio State Softball. I am originally from Conneaut, OH. Spent the last 9 years in North Carolina.
Education/Training: Bachelors of Science (BS) AT - Ohio University
Specialized focus: Evaluation, diagnosis, treatment and rehabilitation of acute and chronic sports related injuries. Functional progressions of athletes return to play programs, Evaluation and management of concussions and concussion related symptoms.

Why do you work at OrthopedicONE: To help everyday people return to performing everyday activities without any limitations or preventions and leading fully functional lives. As well as having an opportunity to educate athletes of all ages and skill levels about the importance of taking care of their bodies if they want to maintain playing soccer at a high level of competition.
